This form can not be submitted electronically. You must print locally.
Send this request and $3.00 cash or money order
for each certified copy of the marriage license to the Clerk of the Court that issued the marriage license to you.
Please provide a Self Addressed Stamped Envelope.
IF YOU CAN NOT RECALL WHICH OF THE ABOVE COURTS ISSUED YOUR MARRIAGE LICENSE SEND THIS REQUEST TO EITHER CLERK OF COURT.
